    Summary:
    The primary responsibility of the Community Health Programs Manager is to support the strategy, development and ongoing implementation of Froedtert Hospital's Community Health Improvement programs. Preparing and monitoring a comprehensive Community Health Needs Assessment (CHNA) and Community Health Improvement Plan (CHIP) is a major responsibility of this position. Reporting to the Executive Director, Community Affairs, this position works in close collaboration with Froedtert Health and Froedtert the Medical College of Wisconsin stakeholders to improve the quality of life in the communities we serve through healthcare programs and services that are measurable, accessible and culturally appropriate; recognizing the greatest opportunity for impact is in Milwaukee's underserved, urban population.

    Education:
    Bachelor's degree in nursing is required. Master's degree preferred.

    Experience:
    Eight plus years experience in public health and/or providing care for vulnerable populations. Experience in health education program design and delivery required. Familiarity with public health data and analysis, as well as population health indicators required. Pediatric care and supervisory experience preferred.

    Licensure:
    Current Registered Nurse licensure by the Wisconsin State Board of Nursing. Current WI driver's license is required. Frequent travel within the Milwaukee metro areas.

    Special Skills:
    Excellent communication, interpersonal and customer service skills necessary to interact with senior hospital staff, clinicians, researchers and diverse patient populations. Ability to manage multiple projects and deadlines. Familiarity with budget, strategic and business plan development. Expertise in Microsoft Office products, including Word, Excel and PowerPoint is required. Proficiency in internet research necessary. Familiarity with database management desired. Must demonstrate commitment to healthcare information privacy as well as the ability to process confidential information in a sensitive manner.
