JOB SUMMARY
This position provides general medical assessment, evaluation and treatment of patients referred to palliative care consultative services in the clinic, home and hospital.  Position is under direction of Palliative Care Physician.
 
ESSENTIAL DUTIES AND RESPONSIBILITIES  
Assess patient data and perform interview, physical examination and begin discussing goals of care within prescribed guidelines and instructions of Physician. Maintain timely consult schedule.  Provide patient and family education to assist patient and family to understand role of palliative care in current health situation. Order, interpret, and evaluate diagnostic tests to identify and assess patient clinical problems and health care needs if required.  Record physical findings, and formulate plan and prognosis, based on patient condition. Coordinate care with Palliative Care Physician and other health professionals; preparing comprehensive patient care plan.  Discuss health care plan and goals of care with Physician partner. Consult with partnering physician and refer to specialized health resources for treatment and evaluation as needed. Assists in developing palliative care protocols and processes.
 
While performing the duties of this Job, the employee is regularly required to stand, walk, sit, use hands to finger, handle, or feel, reach with hands and arms, talk, and hear. The employee is occasionally required to climb, balance, taste, and smell. This position frequently requires close vision, depth perception, and the ability to adjust focus.  The position occasionally requires distance vision, color vision, and peripheral vision. The position requires the employee to exert up to 50 pounds of force occasionally and up to 25 pounds of force frequently.  The position requires occasional travel and home visits.

EDUCATION, TRAINING, & EXPERIENCE  
Graduate of an accredited nursing program.
Meets the requirements determined by the State of Wisconsin or Illinois to perform duties defined as those of a Nurse Practitioner.
 
CERTIFICATION/LICENSURE                                                    
Hold valid state RN license.
